Contents of Arbitration Application

An arbitration application shall specify the following items:

1. The name, gender, age, job, employer, and residential address of the party; the name, organization code, address and contact number, name and title of the legal representative or person in charge of the legal entity or other organizations;

2. Arbitration claim and relevant fact and reasons;

3. Evidences and relevant source, the name and residential address of witnesses.

The application shall be signed or sealed by the applicant or the representative authorized by the applicant.
